Post updated! So far BIOS 1.7.1 seems fine, 1.7.0 gave me 1 unexpected shutdown when sleeping.

The recent 391.01 Nvidia Driver is giving me a huge amounts of issues. Opening Windows Explorer, right clicking on cmd and running as admin etc. have multi second pauses. Persisting across multiple reboots. Rolling back to 390.77 fixes it.

This is the third time in a year Nvidia drivers have had terrible breakage for me (error 43, breaking multiple games, latency on opening apps).

You probably have a serious issue with OS.
Because on my XPS 9650 it's working just great. Without any glitches and problems, no pauses, no reboots, at all.

I'm dubious about this, since I have a fresh install from two weeks back. The issue persists across reboots, and rolling back immediately fixes it. I suspect that the driver may be scanning through a list for optimus, but this is just speculation.

    Currently back to 390.77.

You have to consider to send your laptop to repair/warranty service.

did you try checking on the nvidia control panel if under Desktop "add "run with graphics processor" to the context menu" is enabled? thats usually the cause of the slowdowns

I did not really pay attention to this. Only thing I did was trying 3 full reboots, and paying close attention to the CPU/HDD usage when opening explorer.

But there are now multiple reports:
