What octane fuel do you use and why? Also, where do you fill up and why?

I use 91/93, or Premium at the Top Tier stations, such as Chevron, Shell, BP, etc. :)

Chevron Premium for the IS350 and Eos
    Chevron Regular for the Ridgeline
Chevron, Sinclair, etc. for diesel for the F350

If I can't get to a Chevron, Shell V-Power instead.

    Avoid ARCO like the plague.

I usually just put the cheapest stuff in my gassers, cause I'm cheap that way. Unless its something that runs bad unless on a higher octane. For diesel I look for LSD, but its getting more and more difficult to find so I mostly just go with ULSD. I'm pretty angry about the switch from LSD to ULSD.

Regular, usually from Giant (a local grocery store chain) because of their rewards program.

Ultra Low Sulfur Diesel. I have a Shell station nearby that has 45 cetane, so that's where I usually go. However, often I'm stuck using 40 cetane, since that's what's most common. I'm looking for a station that sells 5% biodiesel. If I can't find one, I'll just keep some B100 and use it as an additive by mixing it with ULSD (to make the final product 5% bio).
