Have you tried any other cards? I know you said it works fine in PEG, but perhaps the issue is with cards vbios...have you tried updating the vbios?

If it is indeed the GPU you could explore baking it, there are a few guides here on...
Would hate to be the bearer of bad news but that looks like the GPU is on its way out...if you have a spare GPU you could check and know for sure.
